All PAISD athletes entering the 7th – 12th grade must have a Pre-Participation Physical Examination annually. As stated by the UIL, NO OTHER Physical Exam Form can be accepted. You can access this form by clicking on the “Download and Print” tab. You may fill out the medical history form online and then print it if you choose to do so. This form must be completed and signed by both the student and parent. The physical form must be completed and signed by a PHYSICIAN and must be dated after May 1 of the corresponding year. Both forms must be turned into the athletic trainer or the student’s coach ASAP.

Physical

1. To access the online forms click the “Electronic Participation Forms” tab. You will get a drop-down list, then click “Port Arthur Online Paperwork”.

a. If you are asked to create a parent account, that is your choice. It is only for you to be able to retrieve information and get notifications, the online paperwork will still need to be completed.

2. Fill out the information requested including cell phone numbers, to enhance communication between you and the athletic department.

3. To sign the document, click and hold your mouse down inside the signature box, which will allow you to create an “Electronic Signature”. If you make a mistake and need to start over click on the refresh icon next to the signature box.

4. Once you have filled out all of the information you will have the opportunity to print the document.

**You will receive a confirmation email once the document has been reviewed by the PAISD Staff**

A STUDENT WILL NOT BE CONSIDERED ELIGIBLE FOR ANY ACTIVITIES AND WILL NOT TRY-OUT, PRACTICE, OR PLAY UNTIL ALL OF THE ITEMS HAVE BEEN SUBMITTED.
